Insurance Claim Forms


Elizabeth Smith Veterinary Practice is happy to complete your pet’s insurance form if you need to make an insurance claim for any veterinary treatment.  You will be expected to settle your account at the time of treatment and then claim the money back from your insurance company.

In occasional circumstances, senior management may agree a direct claim with your insurance company; however, the admin fee, policy excess and any % co-payment will be payable at the time of treatment. Our agreement to submit a direct claim, is in no way confirmation that your claim will be settled by your insurance company and if they decline to cover the claim, you will be liable to pay the full outstanding amount on your account within 14 days.

There is an administration fee of £22 per condition for direct claims and £11 per condition for non-direct claims.

In order to make a claim, please hand in, post or email your claim form to the practice at This email address is being protected from spambots. You need JavaScript enabled to view it. as soon as possible after treatment has started. Please ensure that all the details requested on the front of the claim form are fully completed. Our Veterinary Surgeons will then complete the treatment parts of the claim form and we will then forward it on to your insurance company as quickly as possible.

Alternatively you can inform your insurance company that you need to make a claim and they will send a link to the practice if they prefer to process claims through an online portal.

We are able to re-print and send all relevant financial details so please do not feel that it is necessary to keep any receipts for treatment received. For any new condition, the insurance company will always request that we send the pet’s full clinical history.
